Rhys Hibbert

National Institute for Health and Care Research

ที่มา: <img Subsequent brain scans revealed an 11-millimetre non-cancerous tumour growing on his pituitary gland, a marble-sized organ situated at the base of the skull.

This crucial gland secretes vital hormones regulating growth, metabolism, blood pressure, and body temperature. Unfortunately, it sits in close proximity to the carotid arteries supplying blood to the brain and the optic nerves controlling vision. As Hibbert's tumour expanded and pressed against his optic nerves, it constricted his peripheral vision, leading him to trip over objects while experiencing severe fatigue and dizziness.

Stock photo for illustration only, not from the actual event

11mmPituitary tumour size

25-50%Chance of incomplete removal

0.5-2%Risk of major vessel injury

The surgical procedure required inserting an endoscope—a thin camera mounted on a stick—through the nasal passage to the base of the skull where the tumour lay hidden behind layers of bone and tough membrane. Operating similarly to facial recognition technology on mobile phones, the system instead detects critical and concealed anatomical features.

Developed with funding from the National Institute for Health and Care Research and Google, researchers trained and evaluated the AI system using hundreds of recorded pituitary tumour removals, manually outlining vessels and nerves in each video to teach the software how to recognise key structures. Prof Hani Marcus noted that the tool has been trained on more operations than most surgeons witness in a lifetime, functioning as an expert second pair of eyes that operates in real time. The team's long-term vision is to build a ChatGPT-style assistant for surgeons to consult during complex procedures.

Stock photo for illustration only, not from the actual event

Following the successful operation, Hibbert has experienced steady improvements in his vision every few days over the past eight weeks, with his energy levels fully restored and all prior symptoms eliminated.

Patients also rated their overall recovery and quality of life significantly higher compared to those who underwent traditional treatment methods.

This marks the first major study focusing on Open Dialogue, a methodology for delivering treatment and organizing mental health services centered around patient priorities, seeing the same consistent clinicians, and involving their inner social circle. However, the trial across five NHS trust areas also found that the method did not prevent patients from experiencing relapses, prompting some critics to argue that further research into its long-term effectiveness is still required.

One beneficiary of this approach is Dr Nihad Fathi, who felt his life spiraling out of control as the Covid-19 pandemic took hold in 2020. By the following summer, work-related pressures left him feeling anxious, depressed, worthless, and burdened by the sensation that he was letting his clients down.

Stock photo for illustration only, not from the actual event

During regular sessions, Nihad and his wife Farah sit in a circle inside their east London home alongside two clinicians: consultant psychiatrist Dr Bill Travers and community psychiatric nurse David Adebayo. The sessions facilitate a free-flowing conversation regarding the patient's feelings, desired assistance, and potential methods to achieve it, while also giving their network an opportunity to share observations and personal feelings.

"It enables you to reach a much firmer and deeper understanding of the underlying reasons for the problems and indeed to seek solutions that come very much from the person and their family."
Dr Bill Travers

Led by University College London, the study involved 500 patients who approached mental health services during acute crises, splitting them into two groups where one received Open Dialogue and the other received usual care. Researchers observed no significant difference in relapse rates between the groups; however, among Open Dialogue patients who did require hospitalization, their average length of stay was cut in half compared to the control group.

The Open Dialogue model highlights a paradigm shift from traditional, clinician-centric psychiatric care toward collaborative, network-oriented support systems. By empowering families and maintaining consistency in clinical staff, the approach helps demystify mental healthcare and establishes a secure environment for patients. While resource-intensive elements pose administrative and staffing challenges for standard healthcare frameworks, the demonstrated improvements in patient wellbeing signal a promising direction for future mental health service reforms.

Dr Russell Razzaque, a consultant psychiatrist and clinical lead for the trial, stated that these findings could hold significant implications for how the NHS manages psychiatric care moving forward. Conversely, consultant psychiatrist Dr Sameer Jauhar expressed caution, suggesting that the observed benefits might stem from the extra resources poured into the trial rather than the therapy model itself, indicating that the question remains unresolved.

Source: BBC Health

Researchers from the Cochrane team evaluated 80 major clinical trials encompassing nearly 30,000 individuals, determining that nicotine vapes outperform conventional cessation tools such as patches, gum, and structured support plans.

The findings indicate that nicotine-based vapes raise the likelihood of successfully quitting smoking by approximately 60 percent. Specifically, the success rate climbs from roughly six out of every 100 people managing to stop smoking to about 10 out of 100. Furthermore, these devices proved more effective than non-nicotine vapes, although investigators noted that transitioning to nicotine-free options subsequently could aid in stepping down usage.

60%Increase in smoking cessation success rate

30,000Participants covered across 80 trials

Experts emphasize that while vaping carries certain risks and is not entirely risk-free, it remains significantly safer than smoking traditional cigarettes. Long-term smoking carries a roughly two-thirds mortality rate from tobacco-related illnesses, whereas cessation substantially diminishes the risk of developing severe conditions like heart disease, lung cancer, and stroke.

Source: BBC Health

If high-risk HPV is detected, patients are invited to schedule a follow-up clinical appointment. Trials conducted in London indicate this approach could inspire one in ten women to participate, boosting annual uptake by roughly 400,000 tests, while other regions in the UK explore similar rollouts.

Shifting towards self-sampling options represents a major evolution in preventative healthcare by addressing psychological and physical barriers that traditionally hinder screening participation. By decentralizing the sample collection phase while retaining clinical oversight for positive results, healthcare systems can expand their reach significantly and catch high-risk anomalies much earlier.

Women aged 24 to 64 in England are routinely invited for cervical screenings every five years, or more frequently if HPV is present. Through a combination of adolescent HPV vaccination programs and advanced screening methods, the NHS aims to eliminate cervical cancer by 2040. Health officials maintain that traditional clinician appointments remain the gold standard for prevention.

Source: BBC Health
